Nurses
The District Nursing service provides:
- Health promotion
- Skilled nursing care
- Support and advice
- Education
- Assistance for families and carers to provide care for relatives
A team of District Nurses, Registered Nurses and Health Care Assistants is attached to each GP practice. The District Nurse who has undertaken further training manages the team. The District Nurse is the named nurse who is responsible for assessing any nursing care that you require at home. A 24-hour nursing service is available to provide care at home and in residential homes. District nurses provide some clinic services in various locations.

Referrals are accepted from any source e.g. your GP, patients, family, carers, Social Services, hospitals and Hospices.
A variety of equipment may be available to assist you to maintain your independence at home.
The District Nursing service aims to work together with your family to plan an individual programme of care that is appropriate to your requirements. The District Nurse will visit at regular intervals for reassessment.
