You will qualify if you have severely restricted mobility or are registered blind.

You will be required to provide two passport size photographs and a cheque for £2.00 made out to Kent County Council.

If you are in receipt of the higher rate of Disability Living Allowance mobility you will automatically qualify

Otherwise you can have a blank form sent to you for you to complete.

You can give information over the telephone and a form will be sent partly completed based on the information you give and a letter may be requested by KCC from your doctor to confirm your condition.
